Wow, that's a tough one. Have you talked to him or her about how the condition of his/her body is affecting your sexual desire? Or, have you talked about what is going on that he/she is not staying in shape. The latter might be the best place to begin, as you there may be something deeper (depression, sickness, etc.) going on and you don't want to appear as if you are are self-centered and inconsiderate.

One important law of receiving is to say with sincere gratitude "Thank you." Then wholeheartedly accept what is given to you, knowing that receiving is the gift that you give to the giver. For, if you don't receive as graciously the gift then you deprive the giver of their joy. So, do as your mother taught you and say, "Thank you."
